Octopus (no) Go
Over 3+ years with Octopus, I've mostly excellent service but in August they messed up a change to the Octopus Go tariff big time, which I complained about. The complaint has now been closed, but with no explanation or apology. Although my bill seems to have been adjusted, there's no way for me to check whether the adjustment is correct or not. They sent an email which simply read: "your complaint has now been closed". Ridiculous.
UPDATE: @Andrew, the complaint was raised on my behalf by Octopus, I have no control over what was in the complaint. Yes, it started from a conversation about why I'd been excluded from the free electricity sessions, but the reason for the exclusion was most likely that my tariff change had not been done correctly, hence I didn't qualify?
